With just a $1,000 individual deductible, $20 primary care copays, and 80/20 coinsurance, this plan delivers rich benefits at every level of care. It is ideal for individuals and families who use healthcare regularly and want predictable, low out-of-pocket costs. This plan is available exclusively through Independent Health Agents via Elevate Wellness at large-group pricing not found on the open market.
The Ultra 1000 is offered on the Cigna Open Access national PPO network, providing access to over 875,000 providers across all 50 states with no referrals required. Coverage is effective on the 1st of the month following approval, with a 12-month rate lock from your effective date.
